I am offering my services as a private contractor and wish to perform as a model for Jamin’ Leather. I will be responsible for any insurance required for my health, unemployment or disability. If contracted for modeling I will accept $50 per hour or a flat fee rate for my services. I understand that any test photos and product photos taken are the property of the photographer and Jamin’ Leather and can be used in any artistic medium, including product sales, merchandising or advertising. Test shoot photos are only used to base decisions on which models to work with.
